FUSION ROOM APPLICATION AND FORMS
Please click on the circles to the right to download the Fusion Room FOB Application and the Accident & Waiver Form.
After downloading and completing these forms, scan and send to Tehya at getactive@firstsayyes.com or return to the Celebration Center during these office hours: Monday, Wednesday, Thursday, 8:30-4:30, Tuesday 1:15-4:30, Friday 8:30-3:00 - closed Noon to One for lunch.
